STARR Site Certification Pre-assessment

The STARR Site Certification was created to promote empathy and communication between research site staff and patients, caregivers, and advocacy groups, as well as providing ongoing support for best practices around areas identified by the stakeholders of The STARR Coalition.

The first step in the Site Certification is a very short Pre-Assessment survey.

The Pre-Assessment must be completed for each site location and must include the contact information for the person that will serve as the primary contact for that location’s Site Certification.

This Pre-Assessment survey should only take a few minutes to complete.  Upon completion, you will be directed to a page with information on what to expect next.

Please note: Your first year of Certification is your foundation year – we understand that very few sites will be able to answer yes to these questions. 


Are you actively working with any local advocacy or nonprofit organizations?
Has your site ever done a local community assessment?
Including local demographics and available community resources and services.
Are any of your staff currently involved in a STARR Coalition workgroup?
Are you currently using the STARR 988 program? The program is designed to assist individuals with suicide prevention resources when enrolling in a study who do not meet criteria due to suicidal ideation.
Has your staff ever participated in the Auditory Hallucination Simulation?
